How To Upload Your Script

Our Prescription upload section provides a convenient and easy way for patients and health care providers to submit a copy of their prescription document for quotation and overview by our Pharmacy team. Simply fill in all the fields above and then press “Enquire Now”.

1. Patient Information: Please provide essential patient information such as First Name, Last Name, Email, Contact Phone Number, Address can be added into the comments section, to help us process your prescription accurately and efficiently.
2. Upload Prescription: Click the “Browse…” button to select the prescription document you wish to upload. Acceptable file formats include PDF, JPG, PNG and GIF. We require an edge to edge copy of your prescription.
3. Additional Information: Feel free to include any additional information or special requests related to your prescription order in the provided text box.
4. Recaptcha: Please provide the characters as displayed on the recaptcha box accurately.
5. Submit Enquiry: After reviewing the information you’ve provided and ensuring the accuracy of your prescription uploaded prescription, click the “Enquire Now” button to send your request to our compounding pharmacy team,
6. File Verification: Once your file is uploaded, Pharmacist will overview and verify the documents readability and compatibility. You may receive a phone call with a quotation or should there be any additional information required, a team member will contact you to discuss further.
